Effect of laser therapy on post surgical pain and edema after hallux valgus surgery: a prospective analytical study

ABSTRACT

Objectives: The use of laser therapy has seen a considerable raise in recent years. Its main effects are grouped as antiinflamatory and analgesic. The objective of this paper is to show its effects on the postoperative recovery of patients undergoing hallux valgus surgery.
Patients and methods: An analytic protective study was carried out in 27 patients operated with Austin technique in both feet. Laser therapy was applied in the postoperative period in the right foot (treatment group) and not in the left foot (control group) for 6 weeks, 2 times a week. Postoperatorive edema measuring forefoot contour and pain variables were analyzed at one week, one month and 6 months.
Results: No differences were found regarding the variable edema during the postoperative period. Differences were found regarding pain at one week (mean 2. points - CI95% from 1.136 to 2,863) and at one month (mean 1,62 - CI95% from 0.926 to 2.332) in favor of the right foot compared to the left foot. No differences were found regarding pain variable at 6 months.
Conclusions: The results obtained confirm a beneficial effect of pain reduction with laser therapy in patients undergoing hallux valgus surgery with the Austin technique in the short term. Regarding postoperative edema, no effect was found with the use of laser therapy.



RESUMEN

Objetivos: El uso de la terapia láser ha experimentado gran auge en los últimos años en el campo de la podología. Sus principales efectos se agrupan en antinflamatorios y analgésicos. El objetivo del presente trabajo es mostrar sus efectos en el periodo postoperatorio de cirugía de hallux valgus.
Pacientes y métodos: Se diseñó un estudio prospectivo analítico en 27 pacientes intervenidos de hallux valgus bilateralmente con técnica Austin a los que se les aplicó terapia láser en el periodo postoperatorio durante 6 semanas (2 veces a la semana) en el pie derecho (grupo tratamiento) y no en el izquierdo (grupo control). Se analizaron las variables de edema postoperatorio midiendo el contorno del antepié y de dolor con escala visual analógica a la semana, al mes y a los 6 meses.
Resultados: No se encontraron diferencias en el edema postoperatorio entre el pie derecho y el izquierdo en ninguna de las mediciones postoperatorias. Se halló una reducción del dolor postoperatorio a la semana de 2 puntos (IC 95 % de 1.136-2.863) y al mes de 1.62 (IC 95 % de 0.926-2.332) en el pie derecho en comparación con el pie izquierdo. No se encontraron diferencias del dolor a los 6 meses entre ambos pies.
Conclusiones: Los resultados obtenidos reflejan un efecto beneficioso de la terapia láser en pacientes intervenidos de hallux valgus con técnica Austin en la reducción del dolor a corto plazo. Respecto al edema postoperatorio, no se encontraron beneficios con la aplicación de la terapia láser.
